Health Declaration & Acknowledgement

1. I confirm that:

  • I am in good physical and mental health and am physically capable of participating in the activities of this event.
  • I have not been advised otherwise by any qualified medical practitioner.
  • I will not participate if I feel unwell before or during the event.

2. I acknowledge that there may be certain dangers, hazards, and risks associated with participating in this activity. I understand that not all risks can be prevented.

3. I release, hold harmless, protect, indemnify, and forever discharge the Event Organiser, including its employees and representatives, from all claims, demands, liabilities, costs, and expenses (whether direct or consequential) arising out of or in connection with my participation in the activity.

4. I agree to photographs and /or videos of me being taken and used for event-related purposes.

Medical Consent

In the event of an emergency, I authorise the Event Organiser to arrange for medical treatment as deemed necessary.

I confirm that I have read, understood, and agree to the terms stated above, and that all information provided is true and accurate.

* The Event Organiser reserves the sole and absolute discretion to determine whether the event shall proceed, be modified, or cancelled due to adverse weather conditions or any other circumstances that may compromise the safety, feasibility, or overall conduct of the event. Such circumstances may include, but are not limited to, severe weather, natural disasters, public safety concerns, government regulations, or any unforeseen events beyond the Organiser’s control.
